Who needs to fill out this form?
All graduates attending commencement at Husky Stadium or a school, college or departmental ceremony for which they need cap and gown or parking, will need to fill out this form.
Form Completion Instructions
- Ensure you meet the requirements listed in the section of this website.
- You will need to enter your NetID and password and your name as it appears on your student records.
- Plan ahead and know what you will need for you and your guests at Commencement or other graduation ceremonies you’ll be attending. Perhaps your family and friends will be watching Commencement online in cities around the world.聽 You’ll be asked about that. Will your on-campus guests need parking permits and/or specific disability parking?聽 Will any of them have difficulty walking long distances or climbing stairs?聽 Will they be using a wheelchair?聽 You will be able to request special limited mobility or wheelchair seating, assistive listening devices, and seating in the American Sign Language section.聽 You will also be asked about translation services that will be offered in multiple languages.聽 Think about your guests’ needs in all of these areas. The better prepared you are, the more easily it will be to complete the form.
- Check the requirements for any you are attending. Please be aware that schools, colleges and departments may have additional RSVP requirements other than this form.
- Be prepared to pay online with a Visa, Mastercard, or debit card.
- Choose your pick-up date. Graduates select their pick-up dates (in early May) when they fill out the registration/order form. Cap and gown pick-up will take place at the University Bookstore (4326 University Way NE, Seattle, WA 98105) during normal operating hours. Participants who are no longer living in the Seattle area will be directed via the form to specific pick-up times closer to the date of the ceremony.
- Third party pick-up: If you can鈥檛 make your pick-up date, you may have someone else pick up your order. Find out more on the page.
Changes or Refunds on Orders
- The registration/order form will allow you to make several changes and refunds on your order.聽 You may also be able to obtain extra guest tickets or parking passes when you pick up your order at the University Book Store.
- Graduates will receive instructions on picking up their order at the University Book Store upon submitting their order.
